The Portsmouth Community Foundation considers outcomes - a change in knowledge, skills, or behavior. Your progress report should outline the outcomes proposed in your grant request. Please include information about the survey format and instrument, initial findings, changes in knowledge, skills, and behavior, and the final survey format and instrument.
